You buy the game from a store.
There is an ingame tutorial.
All the "rare" stuff mostly comes at level 60. You get the rare stuff from 40-man instances usually, like Molten Core, Blackwing Lair, etc.. Each class has different tiers of armor, each better than the last. Basically, if you get to level 60, you'll know a lot about all the rare items.
Also, you can get rare epic drops starting at level 35ish off of world drops, yet those are EXTREMELY rare, and barely anyone gets them.
