Default Top features most mmorpgs should have.

I was wondering why most of the seemingly obvious elements of gameplay arent added to most mmorpgs. For example...

-multi button melee
-character growth art(think of fable)

and certain elements that could help prevent people from playing it like its their job

-dependence on other players
-a self sustained economy
-party bonuses
-teamwork bonuses
-more ways to progress rather then just quests and mass murdering
-possibly skills that increased based on your particular playing style

What are everyone else's ideas?
