A question on professional programmers - Game Development

What happens if a pro messes up on a huge graphic which is really messing up the game and stuff? But that graphic can't be repaired, and he did a lot of work. Do you have to start over? And does this happen offen?

Programmers don't tend to do the game art, so I'm not sure what you mean. And I don't see how the art couldn't be repaired. They would just replace the corrupt/error'd image in the game data with the proper one before distributing the game.
