I've always been playing female avatars but the one thing I never do is say I'm a girl lol, That's just stupid. I always end up on a ventrillo in a guild one way or the other so yeah. It's an mmorpg...play whoever the hell you want to play.
Depends on what s/he says. If they talk in a feminine way then I'll assume it's a girl. And vice-versa.

Well, when I see MageMoa's avatar, I assume Bieber is a girl disguised as a boy.
I don't assume, it could be a guy or a girl.
I generally just call them a she if it's a female avatar, male if it's a male avatar. I don't really assume anything about the person behind the computer.
Actually, I don't even refer to them as he or she unless I know, generally just refer to them with a shortened version of their character name.
