Guild Wars 2 and the dynamic event system it has. In the starting village for humans, you can see this especially if the bandits end up poisoning the water supply. Defiance's Dynamic events don't even come close =/
NPCs will even tell you about these disasters on the road before you go past or near it, which makes it feel like trouble.
Anything that is cause-and-effect. If there's a flood, make it seem like there's been a flood. If there's some kind of plague, make it seem like there's some kind of plague. Examples: A city that used to be dry, is now completely submerged and/or is like Venice in real life. Or a city that used to be full of life, is now full of people moaning and dying from the disease, with noticeable amounts of people and NPCs disappearing over time until the disease thing is figured out by the players.
Most of the world events pre-wotlk in WoW. Or the infamous blood plague glitch. That single handedly almost wiped out the entire population of the game.
Lineage II: whenever Blood Sword Akamanah or Demonic Sword Zariche drops.
