I would say it's a bad wire, or sucky soldering at a contact or something, but you say in happens with all your headphones.
In that case, it's probably a problem with your player.

A lot of times its because they get pinched down by where they connect to your device.
I remember my favorite headphones did that, and I bent the wire back at a 90 degree angle right near the part that plugs into the headphone jack and taped it there. It lasted for another few months haha.
Just jerk the wire around lower.

This actually happened with my headphones a few days ago.
Wire just bust, get a replacement where you can.
And if the cloud bursts, thunder in your ear.
You shout and no one seems to hear.

It should also be mentioned that its stupidly easy to splice a different adapter onto the end of your wire.
