Maybe when people get a certain post count + rep then you are allowed to have a custom title. But you could change it only once. That would already eliminate 70% of the users which would otherwise require to be monitored.
And if somebody decides to abuse intentionally then just revert the custom title and never give them a chance to earn the ability to get a custom title again.
