My best advice? Just pick a game and stick to it. If it's been Runescape for 5 years, just play Runescape. It won't kill ya.

Okay so you want an MMORPG that is quest-based without any grind and then in a sentence later you call WoW, Runes of Magic and LOTRO dumb? Lol...

Well then I'll suggest: Allods Online
At least 50% of all the quests don't involve monster killing and the story is good too.

Games without any killing at all:
    Second Life
    Osu!
Mabinogi (if you don't want to kill you don't need to)

And harvest bushes, mark trees, place traps, throw poison on dead corpses so the crows don't eat them, ...

I didn't say I don't want it to be focused on fighting  I said I don't want grinding the same stupid creature about 250 times to level up once.

Played also RuneScape for ~7 years and im just Combat 94. I don't understand how you can consider it low?

Every game is grindable, but you don't need to grind in any game- it's a decision you make. You can exlore the world and enjoy your time there, or grind up to get 'the best'.

Ever tried DDO? Savage2? LostSaga(just ignore the quests)? PokemonWorldOnline? Minecraft? ...

    Well you said:
What that means is I don't want hordes of mindless npcs just standing around waiting to be slaughtered.
Which pretty much includes all MMORPGs with fighting.

I think I have the perfect game for you though: Ragnarok Online
This game is fully about exploring and adventuring. It doesn't have many quests and it is basically grind-only to level but you never fight the same monster over and over, you just fight monsters while adventuring.
This is especially emphasized because it has random spawns, meaning that if you kill a monster it will not respawn where you killed it but somewhere completely else in the current region.
Also monsters will not mindless stand around and wait, they will walk around the world looking for adventurers to kill (unless they are passive, then they just walk around, or they are looters where they walk around and look for loot to steal).
