Gear will, of course, make a difference-- but nothing extreme.
This is assuming you spend the time to gear up in bare minimum blue gear from level 70 dungeons, though; in that case, you won't be extremely underpowered against a player in full tier four or five gear.
Don't forget the most important thing, though: players now have the ability to easily gear up to compete with Tier sixes. You only have to invest a small amount of time weekly in Arena in order to accrue the points to deck yourself out in full season gear. It'll take a while, but the option is definitely there now, so you have absolutely no excuse for having terrible gear, aside from laziness.
Before TBC, this wasn't possible. Tier 3 players were practically invincible, and the only way to even stand a fighting chance was to grind out ranks for Grand Marshal, which took an insane amount of time, and was only ever achievable by the most dedicated gamers, with practically unlimited amounts of free time. The patch that fixed this a great thing, but it was put in place right before the launch of TBC, so it wound up not doing much.
